I am running courier-imap and Evolution as my client. When I create a new account in Evo and set it up to contact my courier-imap server I get INBOX and under that I get Drafts, Sent and trash. Is this the client setting it up or the server?

If it is the client then is there going to be problems when accessing the IMAP server with the same account but using to different clients?
No, there shouldn't be. Each client is different, however, and you may need to manually set some to actually use the folders on the server. Some don't default to using the "Sent" folder on the server, for instance.
The real reason why I am asking is I want a spam directory and and uncaught spam directory under the INBOX. If it is the server that sets it up then I need the config settings to automatically do this when I create a new mail account on the server. If it is on the client side then I need to do it manually.
You could set up the structure that you want in /etc/skel, so that a sane structure is available to all new accounts.
