I just set up a mail list using couriermlm (courier 0.44) and had *exactly* this same problem. I have locallowercase in place to ensure proper local delivery. But after pre-subscribing about 30 people to a new mail list, I found that two of them couldn't get emails in. On a lark, I had one go through the usual subscription process and suddenly she can send mail in... her email address is mixed-case.

Interestingly, in re-reading notes about locallowercase... it seems only to suggest that this will be used for local delivery. So what about addresses of SENDERS? That seems to be the diff here. The address compared to the subscriber list isn't really "local" per se... but I think the semantics suggest that the presence of the file indicates the desire for case insensitive email address handling on a *global*  basis.

as well as [EMAIL PROTECTED]) now, of which only 2 don't work. They
both have capital letters in their domain. 
They ALL receive mail from the list, but 2 cant't post.

Not sure about couriermlm, but for local addresses to be treated case
insensitively, you need to touch 
/usr/local/courier/etc/locallowercase (or
your own equivalent). Don't know if that will help here though.
