respawnlo is set to 5H and there is no respawnhi file.

But maybe the problem is our provisionning system. Everytime a new domain
is provisioned, there is a "courier restart" for the changes on the
hosteddomains file to be in effect.

I guess these forced restarts are the cause. Could it be?

That's exactly what "courier restart" will do.
