I have recently written a purely Perl-based, modular mail filter framework which works 
just as well as the provided perlfilter, but is a lot easier to extend.  Here it is:

For an introduction, read Courier::Filter::Overview.

It should be *very* easy to write a SpamAssassin filter module (without requiring a 
spamd daemon), or a ClamAV filter module (without requiring a clamd daemon or any 
AMaViS stuff).  See the provided filter modules for how easy writing modules is. :-)

I intend to support Courier::Filter on this mailing list.  So if you have any 
questions, bug reports, or ideas, please tell me!
