Update: I've fixed this myself, but thought I would share the solution in case anyone else comes across the same issue in the future.
I neglected to mention that I'm running imapd under supervise as per Shekhar Joshi's installation guide at http://rootshell.be/~shekhar/qmail.html I was editing the /usr/lib/courier-imap/etc/imapd file and restarting imapd but this wasn't actually doing anything as the configuration details are passed in via files in /service/imapds/env/ . I re-ran Jonas Pasche's envconv.pl script to update the environment variables files in the env directory, restarted imapd and all is now working sweetly. Shekhar, if you're reading this, it may be helpful for others if you update your (very useful!) guide to include a mention of re-running envconv.pl any time you update the imapd configuration file.
