This usually means exactly what it says - you have duplicate entries in your 
userdb.  Go back and check the contents of any files in 
the /etc/courier/userdb directory (or in /usr/lib/courier/etc/userdb 
depending on your install).  You've probably got the same account defined in 
multiple files.  

(I sometimes get this if my word processor makes a backup file while I'm 
editing the userdb files. :-)
