So maybe a combination of SA functionality (to do basic MX lookup
testing) followed by a full MX callback? I tracked the callback function
to verify.c in the Exim source. Now just to find time to sit down and
write code. :-)

I think you'd be well advised to use Mail::CheckUser[1], which does exactly what you want. This makes writing a Courier::Filter::Module::CallBack module trivial.
