Do you have an idea of an approach that will work? I haven't been able to get the full picture of how mail flows through courier so I'm having trouble identifying a place to intercept them.

The only workable approach would be to define the mail domain as a local domain, have a global .courier file invoke the spam filter, then forward the filtered message to the other server.
