Your best option would be to use a firewall to segragate the Courier server from the internet, and allow traffic from your MX to pass through.
If that's not an option, I think you can set AUTH_REQUIRED=1 in the esmtpd file, and then set AUTH_REQUIRED=0 for the MX server's IP in smtpaccess.
