The server doesn't care, the client can use the folders in whatever manner it wishes.

I don't quite understand quite how it is all supposed to work.
There's nothing to understand. When enabled, any message that's added to the designated folder is automatically mailed to all addresses listed in the To:, Cc: and Bcc headers. There's really nothing more to say about it.
