Install each cert as $CERTFILE.aaa.bbb.ccc.ddd, where aaa.bbb.ccc.ddd is

1. Shouldn't it be somewhere documented (or is it and I missed something)?

It should.

2. Does it work for IPv6 addresses? What should be filename format then?

Replace aaa.bbb.ccc.ddd with an IPv6 address, in normal hexadecimal representation. Telnet to your own port 25, and see how the IPv6 address gets logged in maillog, and append a period, then everything inside [], to $CERTFILE.
