What OS is Courier on?
We're still on RH 7.3 here, and I disabled IMAP_ENHANCEDIDLE several months 
ago because of response time issues - nothing consistent, but it was quite 
noticable over WAN connections. It's much improved since then, and none of 
the users said anything about no longer having real-time mail notification. 
Most of them are Outlook, but I noticed a difference with KMail too (which 
is odd because, as noted earlier, KMail doesn't even implement IDLE.

FAM's days in Linux distro's seem to be numbered anyway. A Google for 
"groups:courier-users gamin" shows what road is being taken.

From what I recall, it should monitor only what the FAM client (imapd) 
requests, ie. the maildir Inbox. If it's going elsewhere, something else 
may be borked.
