I checked ownership of all the files, and they are all owned by the user/group the courier MTA runs as. I checked the file permissions, and nothing looks out of the ordinary. I admit that I am a little unsure of what I am looking for here.

In your case, I guess wrong permissions. Check out the ownership and modes of all directories and
files related to courier.
