What version of Courier are you using? Did you install the courier-smtpauth rpm package (assuming you are using an RPM based platform)? Courier versions up to 0.47 will require the courier-smtpauth rpm in order to allow SMTP based authentication. 0.48 and beyond will not have a seperate smtpauth package. Also, what backend are you using? Local users, LDAP, MySQL, userdb?
