[courier-users] Header size exceeds policy limit

We have been noticing that some of our internally generated emails
are being rejected with this error:

 

'534 Message header size, or recipient list, exceeds policy limit.'

 

I cannot find anything in the message headers that would cause a
problem.  The entire header is only 418 characters and only two
recipients.  What other conditions can cause this error?  Can it be
caused by anything within the body?
