While this is possible, the errors are getting thrown from the web servers and the user can't change their ACLs from the web server. Now, it's completely possible that the user is say, opening up their account using Outlook Express, changing the ACLs, logging into the website, throwing this ACL, then changing the ACLs back before I can react fast enough to connect via VPN, log into the server, find that user's maildir, and check the ACL file....but I doubt it.

What do you think?

Look at the timestamp of the ACL file when this happens. If it's just been updated, and if you're using NFS, or something similar, then that's the guilty party.
