There's nothing in the default configuration of courier-imap that
installs this file.  The default configuration installs all
configuration files in /usr/lib/courier-imap/etc, and the specific
file here would be /usr/lib/courier-imap/etc/imapd

Sorry... brain must be stuck on /etc... After this wake up call I was able to generate the debug.dat file after a squirrel mail login attempt.

It looks like squirrelmail terminated the connection on its own.
