I would write a script (Perl, PHP, Python, your weapon of choice) to look for the issues Sam mentioned.
After a harsh filesystem problem a long time ago I got some files misidentified as directories (don't ask me how as I don't remember, don't wan't to remember). So besides the issues Sam mentioned you could try to also look for directories in strange places, with strange names (in my case directories named as message files). Beware that it might not be easy to automatically identify problematic directories but some manual intervention might do the trick. Any directory besides first level could trigger your intervention for example.
And obviously don't forget to post your script and results.
