I use courier-mta with MySQL for 10,000+ accounts. It works very well. I also use MySQL for SpamAssassin and SquirrelMail settings so having everything in one place is very convenient for scripting and the like. I've doc'd my setup at http://perlstalker.amigo.net/tiki/tiki-index.php?page=Courier+Mail+Server. The docs are pre-authlib but should still give you a reasonable idea of how things work.
