Thanks for that. I've done as you advised and now have the following
mysql versions installed:

MySQL-client-standard-5.0.17-0.rhel4
MySQL-shared-standard-5.0.17-0.rhel4
courier-authlib-mysql-0.58-1
MySQL-devel-standard-5.0.17-0.rhel4
MySQL-standard-debuginfo-5.0.17-0.rhel4
MySQL-server-standard-5.0.17-0.rhel4

I'll now try to rebuild the rpm from source again and see if it builds
the authlib-mysql rpm properly.
