Yes, Courier-Imap is compiled with mysql support. I tested this by
running authentication from the mysql table, and it authenticates
fine.

I've downloaded the FAMD RPMs and have installed those.

However, when I do a locate (after running updatedb) for fam, the
system doesn't return me anything, so I can't see where I'm meant to
start the service from.

I have portmap turned on from init.d, so that's all right.

I'm just about to download and install gamin now, to see if that helps.
