You must be referring to Courier-IMAP only, since you've mentioned Postfix. Follow-ups to the Courier-IMAP list.
No special account creation process is necessary for simple systems. Use your normal procedures for creating system accounts. Create $HOME/Maildir in each account. Configure Postfix to deliver to maildirs, see Postfix's documentation.
Install courier-authlib. Use PAM authentication, the authpam module. Configure your PAM library for the "imap" service. Use authtest to verify that passwords validate. Do not proceed past this step until "authtest -s imap userid password" succeeds.
Finally, install Courier-IMAP with the default options. Provided that you've verified that authtest works, everything should work.
