Don't modify anything.  Install the correct certfile for the correct IP
address.

I think that I don't get what you mean by $CERTFILE, would I end up having /etc/courier/esmtpd.pem.a.b.c.d; /etc/courier/esmtpd.pem.e.f.g.h::i:j ?

Whichever IP address you see logged, append exactly as it appears to esmtpd.pem
