Thanks for your help Sam. I was not yet familiar with fam. Adding the service sgi_fam/1-2 stream rpc/tcp wait root /usr/local/bin/fam fam to inetd.conf and restarting it resolved the problem.
(I had already installed the fam port before installing courier but missed the man fam page ;)).
