In my SASL smtp.conf I have
SELECT passwd FROM dbmail_users WHERE userid = '%u'
(Which authenticated successfully when I tested SASL on the server)

What you have in SASL is completely irrelevant.  Courier does not use SASL.

README.authmysql.html explains, in detail, how MySQL needs to be set up.
