To test what you said, I did:
Raise quota option (from 5000000000S to 7000000000S) of a test user in 
MySQL backend
Remove old maildirsize file
Send a test mail to this user
Verify quota (with thunderbird) and have 6.518 Go

I run courier-0.56.0 under FreeBSD 6.2-STABLE AMD64x2
