As Sam said: .courier is just for [EMAIL PROTECTED]
For arbitary "-ext", you can use "-default" on every layer. E.g., you can 
use .courier-foo-default for addresses like username-foo-ext for a 
fixed "foo" and arbitary "ext".

Or short: The second one, you must have both in your situation.
