I'd like to set up a "courierfax" chain-like, but with a "couriersms" module that can call a custom "sendsms" script. Of course question (a) is mandatory for this point, cause it's a constraint also for this service.
You can redirect all mail for a domain to a single account, then install a mail delivery script there that does whatever you want it to do.
