What version of courier are you using and where did you get it (compiled
it yourself, distribution packages, etc.)

Have you installed the userdb packages?  On debian squeeze I have to
install the package "courier-authlib-userdb".

What do you see in your logs when you restart authdaemon?

Running Courier 0.65.0 from the debian squeeze repositories, I have
authuserdb and authpam enabled and when I run
"/etc/init.d/courier-authdaemon" to restart the program, this is what I see:
