:[11.22.33.44],25/SECURITY=STARTTLS

(I've also tried :11.22.33.44,25/SECURITY=STARTTLS)

Did you follow all the instructions in INSTALL under "OPTIONAL: Configure the SECURITY ESMTP extension". This is a Courier-specific TLS extension, that requires you to set up your own certificate authority, and install certs signed by your certificate authority.

If all you want is unauthenticated TLS, remove the entry in esmtproutes.
