How embarrassing, reinstalling the new package did indeed wipe out my
previous settings, in particular, esmtpd:ESMTPAUTH. Doh! Sorry about
that, yet again.

How are you doing this? 'make install-configure' should not be clobbering this.

I am still interested to hear in what cases can the extra vhost.address
and vhost.domainname can be used?

It's in the man page. You don't need both if all you want is something that's based on the received IP address. vhost.domain does what vhost.ip- address does, but for the domain's return address, so you can have ipout.domain, etc.

I'll look and see if the authenticated address works. For that to work, though, the logins do have to have the form of "user@domain".
