How does it handle the return code for the content filter if only one of a set of recipients rejects the content?
I identified this gap in SMTP, maybe about ten years ago when I wrote this. At that time, I decided to generate an SMTP 5xx rejection, that's compatible with all SMTP servers, but is structured in a manner that permits a mail server to discern which individual recipients were undeliverable.
So, by default, the sender will think that all recipients were undeliverable, and the documentation specifies that this is the price you pay for enabling this. Translation: do not use this with email addresses you use to subscribe to mailing lists.
Earlier this year, I got an email about someone on IETF waking up and realizing this gap. They were working on something or other that tries to achieve the same goals, and didn't know that I did this a long time ago. Haven't heard anything else since.
