This is really a cool new tool to use by the way... maybe a way for each of us to use within our local communities to gather and then share in a large google doc cloud computing format? ie. in conjunction with the regional cllaboration/ alliances being formed out there?
The form that Tara provides from Cindy is a survey where you can quickly create your own.
