Thanks for your kindly reply,I am working on CPS-3.4.0-3,under Win XP
operating system. The produced html code seems no prolems,as below:

I am confirmed that picture had been uploaded because it's appeared
correctly in pop up window when click its link.

Could you give me more suggestion to resolve it,thanks so much.

I just tried on my bleeding edge CPS checkout and didn't reproduce.  
Which version are you on ? Did you check the produced HTML code ?
Does your browser request the image (check Z2.log for that, or use
LiveHTTPHeaders Firefox extension).
