Can not Install chart of account and same wizard appears two times
(Generate Chart of Accounts from a Chart Template)

Bug description:
If I have tried to install chart of account then it will gives a following
constraint message.
"Tax name must be unique for company"
After that I have faced this error MSG also.
Correct following red fields !
Company (<b>invisible</b>)
Also one problem is Generate Chart of Accounts from a Chart Template
wizard appears two time
