Hmm... must be my css/extensions in our office we have 2 devs who have it broken and 2 who have it ok. Will investigate further, if I know what the actual problem is.

The form view looks correctly in Firefox. Seems like a problem with colspan. Grouping those widgets in: <group colspan="4"></group> produces correct layout in chromium.
