I completely agree something like this should be added, yes there are existing
plugins which already will add a microphone to the top of a users head, but it
would be a good idea to have this as an existing in-game feature. For quite
some time, my community lacked this feature(the SM way), and since we’ve added
it, it has had massive amounts of positive feedback.
As hl said, a new cvar would be even better than smarter than just obeying the
sv_alltalk, as some casual servers, such as a minigames server would have an
excessive amount of microphones above player’s heads when in just casual
conversation.
That’s just my opinion on it, good idea, hope it gets implemented soon.

I agree this would be a very nice feature to have "fixed". The speaker icons

I have noticed weapons floating straight up into the sky from spawn points.
When you first enter the game it's like the bots were there, but are gone
and their weapon floats straight up into the sky.
bot_quota 0 or 1 in local game you will not spawn until next round, this
used to be an old problem too I recall.

It doesn't work for about 2 years at least. I did some research when I
installed my first server for our team in 2013, didn't work since. Only
local game supports it.
