Opening the page in IE6 under Win98SE, the huge gap is dropping the form to my feet - but after a refresh it is coming in the right place... 2. The html-validator reports errors <http://validator.w3.org/check?uri=http%3A%2F%2Fwww.coloradoasphalt.com%2FOnline_Employment.htm&charset=%28detect+automatically%29&doctype=Inline&ss=1&verbose=1#line-728>. Some of them are in relation with the table of the form.
