We usually do both. The word 'menu' under the 3 lines. I believe the three
lines has become well known. It's on the app I'm typing in as a matter of
fact  (without text) however I'd rather be safe and add a text 'label' to
the three lines. But it's certainly not mandatory. I've been seeing a lot
of responsive horizontal scrolling nav without any need for icon
representation.
