New feature: additional vertical alignment options for marginal items ("c" =
vertically centred; "m" = midway between topmost and bottommost baselines).
New feature: option to place a user-specified mark (e.g. an arrow) next to the
relevant line; the mark stays in position even if the marginal item is moved.
Documentation improvements: additional explanation and examples.
