22 CTLS libraries which have already received a funding commitment for Year 4 (July 1, 2001 to June 30, 2002) are listed below.
If your library is listed, you MUST file a Form 486 (which includes the requisite CIPA-compliance certification) by October 28, 2001. Since this date falls on a Sunday, the effective postmark deadline for a Form 486 is really October 27. If you miss the October 28 deadline, you will not be funded for any Year 4 discounts on services received before the Form 486 is submitted. This means you lose any discounts on services extending back to July 1, 2001.
