Here is a message I received from Christopher Hansen, who is

Subject: TeaParty - Form SSA-L996 I am still trying to get the form for the original SSN application. The Form Number is SSA-L996 I was informed that this form has no OMB. I was told by the first person I spoke to that no such form existed. I was also told that they could not send me the forms without a ZIP code. I told her I did not have one and she hung up on me. I called back. (this is a quick call. The wait was only about 30 seconds. Press 0 when you hear the first instruction and it will go to a rep.) The supervisor informed me that I would receive 30 of them in about a week. They informed me that they do not keep evidence of U. S. citizenship in their files. If you call 1-800-772-1213 you can request form SSA-L996 for yourselves. I recommend that you get at least thirty of them and hand them out to all your freedom fighting friends. If we start challenging this all over the USA I think it will be more effective. You should get the new SS-5 form too. You can download the new SS-5 form at: http://www.ssa.gov/online/forms.html
It is very interesting as the restrictions are quite clear. You must PROVE you are a U.S. citizen. That is a tough requirement. On the instructions for form SS-5 page 1 Section Heading "HOW TO COMPLETE THE APPLICATION" #3 3. If you check "other" for CITIZENSHIP, provide a document from the Federal/State or local agency explaining why you need a Social Security number and that you meet all the requirements for the benefit or service except for a number. I called the 1-800 number and asked Mrs. Mathis to please send me a list of the "requirements for the benefit". She informed me that they would research those today and mail those out to me tomorrow. I also asked if if she would please send me a the definition of U. S. citizen since I could not find it in the USC. I felt this was important since I was required to prove I was a U. S. citizen so I think I should know what it means. She agreed and said she would find it. We will see what happens. Christopher Hansen for Congress, Nevada District
