What happens when such a counter wraps? While such a coincidence is unlikely it seems that some timeouts may not work correctly. Is that a valid concern for an application that may stay up for long periods of time?

If that counter wraps, you need to make take that into account when you use that to provide the time for libcurl as libcurl will blindly assume that time always runs forward.
