In lib/file.c, I can look in file_do(), and I see where it constructs a Content-Length header and an Accept-ranges header and a Last-Modified header.

Oh right, I forgot about the synthetic headers we provide there.

Now I finally understand what you are talking about. That code does indeed call the header callback differently.
