I have modified (and attached) the test case project, with the 
@XmlAccessorType(XmlAccessType.PROPERTY) annotation on the complex Exception, 
but the behaviour is the same. The steps to reproduce the problem are:
- Run the server
- Send a soap request. The response is with the Exception elements (doubled, 
another issue?) but the item namespaces are correct
- Request the service wsdl (for example with the browser)
- Send again the soap message. The answer is the same as the first, but with 
the item namespace with xmlns=""
