As has been said before, I'm not in the habit of  verbal "bobbie bashing"...
but....

Ok, so it's posssible for someone to fire a shot accidentally, by weapon
malfunction.......its conceiveable that someone could have a ND.......but
I'm sorry, to fire two, individually, the second presumably while unloading,
can only be incompetence or poor training, or both.....

I think this "experienced specialist firearms officer" has got lots of work
to do, to keep his job !!!!!... and if hes "experienced", God help us with
the new ones....!
I've just seen the News, and the "Sweeny" officer interviewed called it an
AD!!!!

In many years of shooting, both Military and civilian, including Practical,
I have only seen one "true" ND.... a WRAF officer, standing 3 yds to my
left, supposedly having "Made Safe" was advancing from 15 to 10 yds on a
barrack range........after about 3 paces her Browning discharged into the
dirt about 2 yds in front of ME!!!!........the RO had a quiet word with
her.... i often wonder what would have happened if it was me, a squaddie,
that had done it!!!!


Following up on Nick's post about duff Brownings, the other cause of AD's in
Brownings was the firing pin retaining plate, which would crack, jamming the
pin, which could cause at least a "slam-fire"...... at worst a full auto
pistol!!!!
And as for Glocks.... I owned 2, back in happier days, IMHO they are no less
safe than any other pistol.... as long as the basic safety rules are
followed.....
