After my last push, ticket #207 can be closed. Now Cython treats
Py_ssize_t/size_t as basic (but special) C types.

Moreover, the former practice of doing:

cdef extern from *:
    ctypedef unsigned long size_t

would be simply ignored. This is IMHO good for backward compatibility
with previous Cython released and also Pyrex. Of course, in the near
future we issue warnings at CTypedefNode.analize_declarations() (an
not only for size_t !!!).
