Create a PR template
This template instructs contributors to provide the same information as in the
ordinary issue template. This addresses the case where a potential contributor
has not created a GitHub issue for a PR. In that case, the contributor may
have information missing in the description of the PR.
This will help to make it easier for first-time contributors to provide enough
information for their PRs to be reviewed.
