The original patch was for an older version of pymssql that had a bug.
I reported the bug to the authors at the time, so it's very likely that that
patch is no longer needed with more recent versions.
Since then (that was 6 years ago!) I haven't had access to MS SQL. Yes,
I know about Express, but since I switched machines I don't have Windows set up
in my VMs. I'm hoping Jacek can comment on his patched version.
In the meantime, if the unpatched version of pymssql works for you,
keep using that!
