It looks like dt_dev_add_history_item() isn't making the right decision about adding a new item vs. overwriting the last version. I added some debug to my copy and noticed that the behavior happens when the focus_hash between dev and hist is different when it probably shouldn't be.

The last change to this code was back in December, and it worked fine until a week or so ago, so my thinking is that something related to where the focus was changed in the interim. I'll try to run that down this weekend and will patch it if I figure it out. ('Cause it's driving me nuts, too.)
